POSITION TITLE: Community Support Specialist, relief
DEPARTMENT: Developmental Disabilities Services
GRADE LEVEL:

1

STATUS: Non-exempt

 

PRIMARY FUNCTIONS: Provide assistance and instruction in activities necessary to promote and maintain independence for individuals with developmental disabilities living in the community.

SPECIFIC DUTIES:

  1. Assist consumers with various daily living skills, domestic skills, social skills, time management, money management and parenting skills as necessary.
  2. Accompany consumers as needed on recreational activities.
  3. Accompany and advocate as needed for consumers with banking, shopping, doctor appointments, etc.
  4. Maintain accurate records of visits with consumers..
  5. Communicate effectively with consumers with developmental disabilities, service coordinator, supervisor, interagency professionals, community professionals and families.
  6. Provide appropriate interventions to all crisis situations.  Provide immediate notification to appropriate person of any allegation of abuse or neglect.
  7. Attend mandatory training.
  8. Provide transportation, as needed, to consumers and follow safety procedures regarding transportation.
  9. Maintain flexibility in schedule based on needs of consumers assigned.
  10. Other duties as assigned by supervisor.

TITLE OF DIRECT SUPERVISOR: Community Support Manager

QUALIFICATIONS:

 

Education: High School Diploma or equivalent
Experience:

One year practical experience working with individuals with developmental disabilities preferred.

Additional Requirements:
  • Possession and maintenance of a valid New York State driver's license and a driving record considered acceptable by agency's insurance carrier.  If using own vehicle to transport Residents, employee must assure vehicle be reliable, registered and properly insured.  Agency insurance requires anyone whose job description  includes driving responsibilities to be 21 year of age or older.

    Demonstrates behaviors and attitudes which support agency mission philosophy and policies.

    Fosters agency, department and program wide cooperation and team work through use of positive/constructive communication techniques.

    Maintains absolute confidentiality in regards to all records reviewed including consumer records, employee records and billing records.       

    18 years of age or older.

    Ability to work a combination of day, evening and weekend hours, as needed.

    Ability to apply common sense understanding to carry out instructions furnished in written or oral form.

    Ability to effectively communicate with persons with developmental disabilities through words, gestures or pictures.

    Ability to effectively teach, monitor and assist persons with developmental disabilities.

    Ability to climb, stoop, kneel, crouch, reach, sit, finger, grasp and perform repetitive motions.

    Physical ability to lift, grasp, hold or pull up to 50 lbs. when necessary.

    Ability to work both independently and cooperatively as a team member.
