SERVICE COORDINATION |
|||||||||||
We provide Service Coordination to children and adults with developmental disabilities, whether they live in a supervised residential site or in traditional housing in the community. Within Service Coordination we offer assistance in gaining access to needed waivers and state services as well as medical, education, social, and other services. We ensure that the services received correlate with the individual's preferences and changing needs. |

HABILITATION SERVICES |
|||||||||||
Our Habilitation Services provide support in shopping, budgeting, banking, housekeeping, meal planning and cooking, and socialization. Services are based on each person's need and requests and are offered to clients whether they live in an agency site or private housing. Both Service Coordination and Habilitation Services are provided to people with developmental disabilities so that they may remain independent in the community. |

COMMUNITY RESIDENCES (CR's) |
|||||||||||
Our six Community Residences located in different neighborhoods within the Diocese of Rochester are each home to 10 to 12 people and staffed with trained personnel 24 hours a day, seven days a week. |

INDIVIDUALIZED RESIDENTIAL ALTERNATIVES (IRA's) |
|||||||||||
Individualized Residential Alternatives are group living environments similar to our community residences with staffing based on individual needs. |
|||||||||||
SUPPORTIVE APARTMENTS |
|||||||||||
We provide Supportive Apartments for individuals who are able to live somewhat independently in apartments. |
|||||||||||
FAMILY CARE SERVICES |
|||||||||||
Our Family Care Services provide approved, structured, stable home environments, which include the support, guidance, and companionship that are found within a family unit. |

A NOTE ABOUT OUR RESIDENTIAL PROGRAM Any adult diagnosed with a developmental disability may be eligible for services although our main focus is persons with mental retardation. We ask that individuals in our residential services program be able to communicate basic needs, have some daily living skills, and be in general good health, needing no more than outpatient care. Our residential staff provides support in all areas of daily living, for example: health care, service planning and recreation. |
