SERVICE COORDINATION

   Our staff members work to access various services, in the community,
needed by each individual.

INDEPENDENT LIVING SKILLS TRAINING

   We provide guidance, oversight and instruction in areas of daily living.

HOME AND COMMUNITY SUPPORT

   Our home support workers provide assistance, training and supervision
in areas of daily living.

INTENSIVE BEHAVIORAL PROGRAMMING

   We provide assessments, strategies and plans to help our clients
manage any behavioral difficulties.

COMMUNITY INTEGRATION COUNSELING

   By providing counseling to individuals and their families, we assist with
the difficulties and stresses experienced when living in the community with
a brain injury.

NOTE ABOUT ELIGIBILITY

To be eligible for services through the Dept. of Health

Home and Community Based Services Medicaid Waiver
for Individuals with Traumatic Brain Injury, an individual must
have a diagnosis of TBI, be eligible to receive Medicaid and
be between the ages of 18 and 64.
