A traumatic brain injury (TBI) can have any number of causes such as an accident, seizure, stroke, surgery, or tumor and can result in, among other things, memory loss, physical deficits, and intellectual and emotional changes that can affect the individual and all who love him or her. 

In New York State, when a resident sustains a traumatic brain injury (TBI), he or she probably was hospitalized, received rehabilitative services and is now ready for discharge. It is time to make some decisions.

Catholic Charities Community Services is one of the first providers of TBI services under the Home and Community Based Medicaid Waiver for Individuals with Traumatic Brain Injuries (HCBS/TBI) in the Rochester area.  Our experienced staff are dedicated solely to the support of persons living with TBI and their families. The TBI program provides specialized trainings and skills development and was uniquely designed to work with this complex disability.

Our TBI service coordinators will help to:

  • Connect individuals to needed services and supports in the community including, but not limited to Medicaid benefits, medical, housing, financial, vocational and educational.
  • Focus on a person’s strengths, abilities and supports needed to live independently in the community
  • Work collaboratively with all supports to ensure effective, person-driven services of the highest quality
  • Secure resources to assist with environmental modifications and assistive device technologies such as personal emergency response systems 
  • Review the service plan, assess progress toward the stated goals and guide the individual and his/her loved ones on a path to the individual's independence with dignity
